Assumption of Risk, Waiver of Liability, and Release Agreement

KC Panthers Development – Youth Athletic Training Program

PLEASE READ CAREFULLY BEFORE CHECKING THE BOX.

By checking the box and completing this purchase, I confirm that I am the parent or legal guardian of the minor participant and that I agree to the terms below on behalf of myself and the minor participant.

1. Assumption of Risk

I understand that participation in athletic training, conditioning, drills, instruction, games, and related activities involves inherent risks, including but not limited to muscle strains, sprains, fractures, concussion, illness, permanent injury, paralysis, or death. These risks may arise from physical activity, use of equipment, facilities, weather conditions, instruction, or the actions or negligence of other participants.

I knowingly and voluntarily assume all risks, known and unknown, associated with the minor’s participation in KC Panthers Development activities.

2. Voluntary Participation and Fitness

I affirm that the minor is participating voluntarily and that I am solely responsible for determining that the minor is physically and medically able to participate. I acknowledge that KC Panthers Development does not evaluate or guarantee participant fitness.

3. Waiver and Release of Liability

To the fullest extent permitted by Missouri law, I hereby release, waive, discharge, and hold harmless KC Panthers Development, including its owners, officers, directors, coaches, employees, contractors, volunteers, agents, and affiliates, from any and all claims, demands, damages, losses, liabilities, or causes of action, whether arising from negligence or otherwise, that relate to or arise from the minor’s participation in the program.

4. Covenant Not to Sue

I agree that I will not file, initiate, or participate in any lawsuit or legal action against KC Panthers Development or any released party for any injury, illness, damage, loss, or death related to the minor’s participation in the program.

5. Medical Authorization

In the event of an emergency, I authorize KC Panthers Development to seek medical treatment for the minor participant. I understand and agree that I am financially responsible for all medical costs incurred.

6. Insurance

I understand that KC Panthers Development does not provide medical or accident insurance for participants. I confirm that I am responsible for maintaining adequate health insurance coverage for the minor.

7. Photo and Video Release

I grant permission for KC Panthers Development to photograph or record the minor during program activities and to use such media for promotional, educational, or marketing purposes without compensation.

8. Governing Law

This agreement shall be governed by and interpreted under the laws of the State of Missouri.
